Our CHW service is the cornerstone of our commitment to supporting families throughout the perinatal period. CHWs are trusted, compassionate professionals who often live and work in the communities they serve, building authentic relationships and delivering community-responsive support to women and expectant families. By bridging the gap between families and healthcare systems, CHWs ensure that everyone receives the resources, information, and advocacy they need to achieve optimal health outcomes.
Our CHWs meet families where they are, offering consultations (virtual or in-person), and ongoing check-ins tailored to each family’s preferences and needs. They provide guidance on
navigating prenatal and postpartum care, connect families to vital resources such as nutrition support, housing assistance, and mental health services, and offer a listening ear through every
challenge and milestone. With a deep understanding of the social and heritage factors that shape health, our CHWs are here to make sure no one feels alone during their journey.
Our CHWs work collaboratively with other support team members including doulas, healthcare providers, and local organizations to deliver coordinated, wraparound care that honors each
family’s unique story. They advocate for your voice to be heard and your choices to be respected at every stage, from planning a healthy pregnancy to navigating postpartum adjustments.
